We will need to know where the patient is currently located and where the patient will be transported to, plus the physical condition of the person. Please also tell us about the person’s medical condition, such as whether oxygen is required, special diet, feeding tubes, and other pertinent information. We also need to know the reason for the transfer and the names of the passengers who will be accompanying the patient.
